Common pitfalls
Assuming the patient's cultural background determines their views. Using family members as interpreters (loss of confidentiality, distorted translation). Imposing a biomedical model before understanding the patient's framework. Performative cultural references that miss the actual patient's position.
Red flags / escalate: Family member translating actively shapes what the patient says. Patient expresses fear of involvement with mental health system due to legal/immigration concerns — address directly. Concerns about confidentiality that may reflect specific community considerations.
Documentation
Note language used and interpreter status. Note patient's explanatory model when it shapes care decisions. Note any specific cultural or religious considerations relevant to the plan.
Real-world reality: The five-minute close requires the discipline to stop exploration even when the patient is engaged. Most clinical encounters that go poorly do so because the close was sacrificed for an extra exploration item.

Maintain eye contact with the patient, not with the interpreter. Speak in short segments. Verify understanding through teach-back.
Specific concerns warrant deliberate screening. Immigration status concerns may suppress disclosure of substance use, intimate partner violence, or other sensitive topics. Religious community considerations may shape what the patient is willing to discuss or accept as treatment. Legal status may affect comfort with documentation. Asking — non-judgmentally, after rapport — opens information the patient may not volunteer.
Avoid performative cultural references, assumptions about the patient's views based on demographic category, and the trap of cultural "competence" framed as fact-mastery. Each patient is the expert on their own culture; your job is to learn from them, visit after visit.
